1.4 Medical Certificate indicating the students medical condition (fit for school)
1.5 Photocopy and Original Report Card of last level completed (if w/ previous schooling)
1.6 Photocopy of the ACR (Alien Certificate of Residency) and passport for foreign students

*After submission of the required documents, the students will be scheduled for an interview.

Grade School Students:

1. Processing Fee for admission of P300.00. This includes the interview of the child and parents, and diagnostic testing fee and processing
